These Terms & Conditions (“Terms”) govern your access to and use of the My Health Passport (“MHP”, “we”, “us”) websites, applications, and screening services. Please read them carefully. By using MHP or consenting to a child's screening, you agree to these Terms.
1. Acceptance of Terms
By accessing or using the platform you confirm that you have read, understood, and agree to be bound by these Terms and by our Privacy Policy. If you do not agree, please do not use the service.
2. Eligibility
You must be at least 18 years old and capable of entering into a binding contract to create an account or provide consent. Where you act for a child, you confirm you are the parent or lawful guardian and are authorised to provide consent on the child's behalf.
3. Our Services
MHP provides preventive health screening across multiple domains, an expert-reviewed Health Passport summarising results, and optional guidance, programmes, and appointments. Screening is delivered in partnership with schools and trained professionals.
4. Not Medical Advice
MHP screening is a wellness and early-identification service. It is not a diagnosis, treatment, or a substitute for professional medical advice, and using the platform does not create a doctor–patient relationship. Results and recommendations are intended to help you decide whether to seek further evaluation. Always consult a qualified healthcare provider for medical concerns, and seek emergency care where appropriate.
5. Accounts & Security
You are responsible for the accuracy of the information you provide and for keeping your login credentials confidential. Notify us promptly of any unauthorised use of your account.
6. Payments, Subscriptions & Refunds
Paid programmes and services are billed through our payment partner (Razorpay). Prices, packages, and inclusions are as displayed at the time of purchase and may change prospectively. Taxes apply where relevant. Refund and cancellation terms for each programme will be made available at purchase and are subject to our applicable refund policy.
7. Parental Consent & Child Participation
Participation in screening is voluntary and requires verifiable parental or guardian consent. Consent may be withdrawn at any time by informing the school or contacting us; withdrawal does not affect processing already carried out lawfully before withdrawal.
8. Acceptable Use
- Do not misuse, disrupt, or attempt to gain unauthorised access to the platform.
- Do not upload unlawful, harmful, or infringing content.
- Do not access data relating to children or families you are not authorised to view.
9. Intellectual Property
The platform, its content, design, and the Health Passport format are owned by or licensed to MHP and protected by applicable laws. You retain rights to the personal information you provide; we use it only as described in our Privacy Policy.
10. Disclaimers & Limitation of Liability
The service is provided on an “as is” and “as available” basis. To the maximum extent permitted by law, MHP disclaims implied warranties and shall not be liable for indirect, incidental, or consequential damages. Nothing in these Terms limits liability that cannot be excluded under applicable law.
11. Indemnity
You agree to indemnify and hold MHP harmless from claims arising out of your misuse of the service or breach of these Terms, to the extent permitted by law.
12. Governing Law & Jurisdiction
These Terms are governed by the laws of India. Subject to applicable law, the courts at Hyderabad, Telangana shall have jurisdiction over any disputes.
13. Changes to These Terms
We may update these Terms from time to time. Material changes will be notified through the platform or by email, and the “last updated” date above will change. Continued use after changes take effect constitutes acceptance.
